The Case Manager can use this screen any time additional space is needed to elaborate on information from Pre-Discharge, Post-Discharge or 30-Day Follow-Up tabs. The date and time of each comment will be posted, and a sequential number assigned to each comment, as illustrated above. “No. 1” will always be the first comment added by the Case Manager, “No. 2” second, and so forth. One use of this section is to specify key social services received by the client since discharge that were provided by organizations other than the CMHC, e.g., job training, housing assistance, etc. Below are examples of information which may be entered by the agency.
This area is used by the jail to view comments about the detainee which were entered by community agency staff. The comments may be helpful in joint planning for discharge. Below are examples of information which may be entered by the agency.
