This screen has two sections:
First section shows the major program codes for DMH-funded service(s) provided by community agency. A checkmark in the box indicates that the service was received by the detainee within the last 24 months. In this case, CRISIS services were utilized by the detainee.
Second Section lists the DSM IV diagnosis for the detainees reported by the agency in ROCS.
